Oral bacteria do not metabolize starch per se, but it is plausible for starch to be cariogenic if hydrolyzed to sugars intraorally by amylase.

Higher-quality studies need to be designed and implemented to enable firm conclusions to be drawn regarding any relationship among different types of starch and starch-rich foods (e.g., whole grain vs. non–whole grain) on oral cancer and periodontal disease and to confirm the current observations pertaining to risk of dental caries.This systematic review shows evidence that RDS intake but not total starch intake is associated with increased risk of caries.
